In order to get a quote or enroll for coverage you'll need your Employee ID and your PIN to login the system. 

If you have previously logged in, use the unique PIN you created. 

Logging in for the first time?  Use your temporary PIN which is the last 4 digits of your Employee ID + your 4 digit birth year.  (i.e. 33331988)

All associates will be required to establish a unique PIN with their first login to use going forward.

What are the guidelines to create my unique PIN?

-  Your PIN requires a minimum of 4 characters and cannot exceed 30 characters

-  Any combination of letters, numbers, and symbols are accepted

-  Your PIN cannot be the same as your Employee ID

 

Where can I find my Employee ID?

-  Check your email inbox.  Your Employee ID is provided within the LTC program emails, or...

-  Log into my.Ascension.org (click here)

-  Select myHR > My Info > My Personal Info > Personal Information.

 AMITA Associates:

-  Log into my.amitahealth.org (click here)

-  Select > HR Information > Personal Information
